Toolkit

AGM membership form.

A paper form for the AGM, or for a night when someone cannot pay on a phone. It does not replace the join link. The treasurer types the details into the club record, or the member pays online later.

What it is for

Print a few copies for the table at the door. Ask for the name, date of birth, contact, address, membership and how they paid. A parent or guardian signs for a child. Photo consent is a yes or a no, not a blank.

subs.ie does not receive this paper. Nothing is filed with a county board from the form itself. When you are back at a computer, add the member on the join page or record the payment the treasurer took. Cash, a bank transfer and a cheque have no online fee.

After the meeting

  • Record each cash, transfer and cheque payment so the paid list matches the forms.
  • Send the join link to anyone who ticked pay online later.
  • Keep the paper for your own committee, then follow whatever retention the club already uses.
  • Shred or delete contact details you do not need once they are on the club record.

The fields match what a club stores for a member: name, date of birth, contact, address, membership and payment. Exports and records are on Features. The fee is on Pricing. Handing the forms to next year's treasurer is part of the treasurer handover.

Questions

Does this form sign the member up on subs.ie?

No. It is paper. Someone still types the details into the club, or the member uses the join link. subs.ie does not collect the paper forms.

Can we print it?

Yes. Use the print button, or download the form and print that. Either one is the same fields.

What about a child?

A parent or guardian fills in their own name and signs. The member's date of birth is on the form so the age grade can be worked out later.

Where do we record the cash?

On the club's payment list, as cash, a bank transfer or a cheque. Those payments have no online fee. The pricing page shows the fee for an online payment.
